The City of Attleboro Council on Aging seeks an outreach worker/case manager, who working under the direction of the Council on Aging Director, identifies the needs of older adults, provides case management and assistance accessing services, advocacy and education to enhance the quality of life for Attleboro’s older adults and caregivers.   

Duties include conducting needs assessments at the Council on Aging or in the homes of older adults to identify services which meet their specific circumstances.  To provide referrals for services and case management to identified older adults and/or caregivers.  Provides advocacy and education for clients and the elder population.  Coordinates services and programs with area agencies serving older adult and disabled populations.  Maintains client records in a confidential manner and adheres to the state’s record retention schedule protocol.  Collaborate with various community stakeholders which meet the needs of older adults.  Utilizes MySeniorCenter, in real time to capture data that creates detailed case management notes that chronicle case management history and provides a statistical view of the impact Outreach has on Attleboro’s older adults.  Assists clients in achieving self-sufficiency and/or their ability to remain in their own home.  Performs related work as required by the COA Director.

Requires a thorough knowledge of aging issues and resources and sensitivity to the needs of older adults, including an understanding of medical, social, and psychological issues prevalent in the older adult community.  Familiarity with federal, state and local regulations and programs relative to the aging population.  Ability to conduct assessments, perform crisis management, disseminate information to appropriate individuals and agencies.

Requires a Bachelor’s degree in social services or related field and one or more years’ experience as a case manager or any equivalent combination of education and experience. Successful candidate must have a valid drivers’ license, Shine Counselor Certification (or willingness to obtain within first year of employment), and CPR/AED certification.
